City of College Park Volunteer Waiver & Release
As a non-paid Volunteer, I understand that I could be placing myself at risk of exposure associated with the COVID-19 Virus.
I hereby state and affirm that in consideration for a non-paid Voluntary service effort, I agree to and /or hereby do release and agree to hold harmless the City of College Park and its employees from any and all claims, suits, demands, liability, damages, costs, fees, injuries, contracted disease(s) that I and any person(s) in my company may experience which is in any way related to my Voluntary service.
I understand that I will be using my individual property as a non-paid Volunteer, and I agree to and/or hereby do release and agree to hold harmless the City of College Park and its employees from any and all claims, suits, demands, liability, damages, costs, fees, injuries, or damage that I and any person(s) in my company may experience which is in any way related to my Voluntary service.
If needed, I agree to medical treatment on my behalf and that of any person(s) in my company which is in any way related to my voluntary service. If I am injured or require medical attention due to my participation, I agree that I am solely responsible for any and all costs related to such medical treatment, transportation, and evaluation. I fully release, discharge, and hold harmless the City of College Park and its employees.
The City does not assume any obligation to provide a Volunteer with compensation or any other financial assistance, including but not limited to medical, health or disability assistance, benefits, or insurance in the event of Volunteer’s injury, illness or death or damage to Volunteer’s property. The City does not guarantee that you will be provided with a service opportunity.
With my signature I agree to the possibility of a background investigation for myself and others in my company during my service.
With my signature I agree to abide by all the City of College Park safety guidelines and rules for participation.

For your minor child to become a Volunteer with The City of College Park, we must receive your written consent.
We encourage you to review your child’s Volunteer information and become familiar with the Volunteer position and your child’s supervisor.
I understand that he/she/they:
-May participate in an orientation & training program prior to placement in the Volunteer position.
- Will commit to work a schedule of Volunteer hours.
-Will be expected to be faithful in honoring his/her/their volunteer commitment.
-Will be expected to abide by all the City of College Park safety guidelines and rules for participation.
